TEAM REGISTRATION FOR THIS EVENT STARTS TUESDAY, JULY 9th, AT NOON! Registration ends on Tuesday, July 23rd.
This is the official registration form that must be completed by each Team that wants to compete. Each Team must designate a Team Leader who will be the contact person for the Team. By completing and submitting this registration form each Team agrees to the rules of the competition and agrees to the waiver of liability. The rules and liability waiver will be emailed to each Team.
Important: We’re hoping to have 18 teams participate. If more than 18 register, then the selection of the participating teams will be based on first-come, first-served. In other words, get your team together quickly and register as soon as possible after noon on Tuesday, July 9th so you will be included.
Some notes about this form:
1. If any team member is under 18 years of age, a parental consent form must be completed. Please note any team member under 18 in the field for Team Member Names and we will email you a parental consent form.
2. We will try to accommodate each Team’s preferred Playoff Date, but we cannot guarantee it. If there is a date your Team absolutely CANNOT attend, you can let us know in the field below.
3. In the field “Fun/Interesting info about your team”, we’re looking for some info we can use to introduce your team, e.g. how did you come up with the Team Name?, how do you all know each other?; basically some brief info to describe your team.
